It is the question I am asked most often, usually phrased as a hope: what amount is safe? The honest answer is that there is no single number that is safe for everyone — and understanding why is far more useful than any threshold I could quote.
I am a liver transplant and HPB surgeon, so I meet alcohol-related liver disease at its end stage. That gives me a particular perspective: almost every patient I see at that point had years — often decades — when the damage was quietly building and entirely treatable, and nothing hurt. This piece is about that window, not about lecturing anyone.
There is no safe threshold, only a gradient
Risk from alcohol is not a cliff you fall off at a certain number of drinks. It is a slope that steepens. Two people drinking identically can end up in very different places, because the threshold shifts with:
- Biological sex. Women have less of the gastric enzyme that starts breaking alcohol down, so more reaches the liver, and a lower body-water ratio concentrates it further.
- Body weight and composition. Less mass means a smaller volume for the alcohol to distribute into, and a heavier metabolic load per drink.
- Genetics. Variation in the enzymes that clear alcohol means some people accumulate its toxic intermediate faster than others.
- Nutrition. A poor diet deprives the liver of what it needs to repair itself.
- Existing liver disease. This is the big one, and the reason the next section matters.
When the liver processes alcohol it produces acetaldehyde, which is toxic to liver cells. Drink faster than the liver can clear it — roughly one standard drink an hour — and that compound accumulates and injures tissue directly.
What a "standard drink" means in pegs
Global guidance talks in units, which is useless if you pour in pegs. A clinical standard drink is about 10–14 g of pure ethanol. Translated:

Two large pegs in an evening is four standard drinks, not two. A single large bottle of strong beer is closer to three and a half. People routinely underestimate their intake by half simply because of how it is served.
The damage is silent, and that is the problem
Alcohol-related liver injury moves through stages:
Fatty liver (steatosis). Fat accumulates inside liver cells. This begins after even a short period of heavy drinking, and it is substantially reversible.
Alcoholic hepatitis. Inflammation sets in. It can smoulder at low grade for years, or present acutely and dangerously after a heavy spell.
Fibrosis. Persistent inflammation lays down scar tissue, which starts to restrict blood flow through the liver.
Cirrhosis. Scarring becomes architectural and permanent. The liver stiffens and loses function. This does not reverse.
Here is the crucial part: the functional tissue of the liver has no pain nerves. Pain receptors sit only on the outer capsule. You can progress from a healthy liver through fatty liver into early fibrosis and feel completely well. Most people discover it on a scan or blood test done for something else entirely.

Why this hits Indian patients harder
This deserves saying plainly, because it changes the arithmetic.
Metabolic fatty liver — the kind unrelated to alcohol, driven by insulin resistance, central weight gain and refined-carbohydrate-heavy diets — is very common in India. A great many people already have some fat in the liver without drinking at all.
Add alcohol to a liver that is already metabolically stressed and the effects are not simply additive. Damage progresses faster, and amounts that might pass as low-risk elsewhere can drive fibrosis here. Combined with a drinking culture that leans towards heavy episodic sessions rather than small steady amounts, this is a genuinely higher-risk combination — and it is why I am cautious about quoting Western "safe limits" to patients in Chennai.
Signs the liver is already struggling
Early and easy to dismiss: persistent fatigue, loss of appetite, mild dull discomfort under the right ribs, nausea after fatty meals. These get attributed to stress and overwork, and usually not investigated.
Signs that need prompt assessment:
- Yellow eyes or skin, dark urine, pale stools — see what causes jaundice in adults.
- Swelling of the ankles, or a distended abdomen — fluid accumulating because the liver is not making enough albumin.
- Easy bruising or bleeding gums — clotting proteins are made in the liver.
- Confusion, forgetfulness, slurred speech, unusual drowsiness — toxins the liver should be filtering reaching the brain. This needs urgent medical attention.
These overlap heavily with the warning signs of cirrhosis, which goes into them more fully.
What happens if you stop
This is the genuinely hopeful part, and it is worth being precise about it rather than vague.
- First few days. The liver finishes clearing the alcohol; oxidative stress begins to settle.
- Two to four weeks. In simple fatty liver, fat begins to clear and liver size and function start normalising.
- Three to six months. Inflammatory markers typically settle. Early fibrosis can partly remodel.
- Beyond a year. Established cirrhosis will not reverse — but abstinence halts progression and substantially reduces the risk of liver failure and liver cancer.
So the honest split is: early damage often recovers well, late damage stops getting worse. Both are worth having.
One important safety point. If you drink heavily every day, do not stop abruptly on your own. Alcohol withdrawal in a physically dependent person can cause seizures and delirium and is occasionally fatal. That is not a reason to keep drinking — it is a reason to stop with medical support.
How to actually get your liver checked
- Liver function tests (LFTs). Useful, but understand the limitation: a normal LFT does not rule out liver damage. In slow chronic disease, enzymes are often normal despite real scarring.
- Abdominal ultrasound. Shows fat, liver size and structural change.
- FibroScan (transient elastography). Measures liver stiffness, quantifying fat and fibrosis without a biopsy. This is the test that most often settles the question.
- Platelet count and clotting profile. Reflect the liver's synthetic function.
If you have been drinking regularly for years, a normal blood panel from a health package is not reassurance. Ask specifically about fibrosis assessment.
When it becomes a surgical question
If cirrhosis decompensates — variceal bleeding, fluid that no longer responds to medication, recurrent encephalopathy — medical management alone may not be enough, and transplantation becomes the definitive treatment.
Transplant programmes require a documented period of abstinence, typically at least six months, together with multidisciplinary medical, psychological and nutritional assessment. That requirement is not a moral judgement: it allows recovery from the acute effects, confirms fitness for major surgery, and protects a scarce graft. Most people reading this are nowhere near that point — and the entire purpose of this article is to keep it that way.
